Availability of IRS Form 1095-B

The IRS no longer requires us to automatically mail paper copies of Form 1095-B to all individuals. This form provides proof that you and your family had health insurance during the tax year.

You are not required to attach this form to your tax return, but you may want it for your records.

 

How to Request Your Form

If you would like a copy of your Form 1095-B, you may request one free of charge through any of the following methods:
